Mr. Wallace joined St. Johnsville as Administrator in 2013. He brings to the Center over 20 years of post-acute care experience. Mr. Wallace has a passion for senior care and is a life-long learner. His career in health care began when he served as a transportation aide in 1995 and later earned his CNA. He attended Madison-Oneida BOCES and earned a License as a Practical Nurse (LPN) in 1998. Mr. Wallace earned a Bachelor of Science in Health Administration / Long-Term Care in 2011 and a Master of Health Administration / Education in 2013 from the University of Phoenix. Mr. Wallace is a recipient of the 2015 and 2016 American College of Health Care Administrators Eli Pick Facility Leadership Award. Mr. Wallace is an active volunteer in several community organizations including the New York State Health Facilities Association's Clinical & Quality Committee, Education Committee, and Hospital DSRIP Clinical Governance Committee. He also serves as Treasurer of the Kayuta Lake Improvement Association, and is a member of the American College for Health Care Administrators. Mr. Wallace resides in Remsen (Kayuta Lake), New York.
Ms. Wentworth holds an AAS in Human Services and has worked in the human service field in the community for over 25 years. Her background includes working with children and adults with developmental disabilities and mental health needs. Ms. Wentworth has a great deal of experience in long-term care. Some of her work includes assisting people to connect with community resources and the necessary services to meet each resident’sneeds. She served as a local elected official for 8 years and continues to serve on many boards and committees in her community. She resides in Gloversville with her husband and family.
Mr. Miller joined St. Johnsville Rehabilitation & Nursing Center in 2014. He earned an Associates Degree from Herkimer County Community College, and a Bachelor of Arts in Sociology at SUNY Oswego. Mr. Miller is originally from Brooklyn, was raised in New Jersey, and moved to Central New York in 1999. He resides in Canajohaire with his family.
Mrs. Dygert has been a long-standing employee of St. Johnsville for the past 26-years. She is a St. Johnsville native and attended St. Johnsville High School. Mrs. Dygert worked with the original founders of St. Johnsville Rehabilitation before the building was built in 1989. She is an active community member, and brings many years of on the job experience in the area of health care financing. She lives in St. Johnsville with her family.
Ms. Harding has over 38 years of health care management experience and has been part of the St. Johnsville Rehabilitation team for the past 26 years. Ms. Harding has both acute and long-term care experience. She obtained her Bachelor of Arts from the State University College at Geneseo in New York. Ms. Harding is a lifelong resident of St. Johnsville. She enjoys spending time with her family and son, who is a United States Army Ranger.
Prior to joining St. Johnsville Rehabilitation & Nursing Center in 2013, Ms. Thomas was VP of Operations for a boutique investment firm with offices at Wall Street, New York and Ft. Lauderdale, Florida, and back office operations upstate. She has spent many years as a manager in Banking and Finance and also owned and operated a family business. Ms. Thomas is a graduate of Hamilton College and lives in Forestport, New York with her husband.
Ms. Hoffman started her career at St. Johnsville in 1990. She first earned her CNA and then attended Herkimer BOCES and obtained her LPN, returning to the facility in 1994 as an LPN. She worked as a charge nurse until 2002 when she was asked to be our MDS data collector, then our MDS Coordinator. Ms. Hoffman graduated from St. Elizabeth College of Nursing in Utica, New York where she earned her RN. She brings to St. Johnsville over 25 years of long-term care experience.
Ms. Vair holds a Master of Music in Music Education from the Fredonia School of Music as well as a Master of Science in Nursing from Sage Graduate School. She is an Adult Nurse Practitioner. Before becoming a Nurse, she spent 14 years teaching vocal music in public schools specializing in Musical Theater. Ms. Vair is the Staff Educator, Infection Control Coordinator, and also is the Coordinator of Employee Health. Her experience in nursing ranges from Maternal and Child Health to Intensive and Progressive Care, Cardiopulmonary Rehab, and Community Health. She is a former Adjunct Nursing Instructor at Russell Sage College, and has held National Certification in Progressive Care Nursing as well as Palliative Care Nursing. At the end of her graduate studies, Ms. Vair was a recipient of the Helen Riewarts Matthei Award for Graduate Nursing Research. Her Thesis subject was “How do Nurses Initiate Dialogue Regarding End-of-Life Options with End-Stage Congestive Heart Failure Patients?” She is now the Minister of Music at the Sprakers Reformed Church and the Fort Plain Reformed Church. She is also the Director of a women’s singing group based out of Johnstown called the Dreamcatchers. She resides in Johnstown with her Husband David, her son Brian, and her very spoiled rabbit named “Bunny”. Her daughter and son-in-law live in Virginia.
Dr. Curri is a graduate of Utica College of Syracuse University where he earned his Bachelor of Science in Health Studies in 2006, and his Doctorate of Physical Therapy in 2008. Dr. Curri is a New York State licensed physical therapist. He is a Certified Lymphedema Therapist through the Academy of Lymphatic Studies and Wound Care Certified through the National Alliance of Wound Care. He has extensive experience in the areas of evaluation and treatment of musculoskeletal injuries, post-surgical therapy, lymphedema care, and wound care management. Dr. Curri is a Captain in the United States Army Reserves and resides in Utica, New York with his family.
Ms. O’Bryan received her AAS in Applied Science and Recreation at Herkimer County Community College. She has worked in health care as an Activities and Recreation Director over the last 20 years. Her specialties include working with seniors and adults requiring short-term rehabilitation. She has also served as a member on the Annunciation Church Board. At St. Johnsville, Ms. O’Bryan created a Pet Policy, and advocates enthusiastically with families to ensure a happy environment for the residents.She also works with the community to seek volunteers for promoting excellent coordination with this health facility. She strives to design a resident and family-oriented activity program that promotes the dignity, autonomy, and independence of every individual. Outside of work, she mentors college students seeking health related degrees for Human Services. Ms. O’Bryan resides with her family in Herkimer, New York.
Ms. Cogan joined St. Johnsville Rehabilitation & Nursing Center in 2000 after obtaining her Associates Degree in Applied Science from Herkimer County Community College in New York. She is a New York State Certified Occupational Therapy Assistant. She started her career 24 years ago as a Certified Nursing Assistant working in various health care facilities. She is skilled in the areas of Worker's Compensation Programs, OSHA, and Workplace Safety. Ms. Cogan has led the facility in 7 consecutive SHARPS Awards of Distinction through the Department of Labor, which exemplifies the centers commitment to workplace safety. She resides in St. Johnsville with her family.
Ms. Holmes joined the St. Johnsville Rehabilitation & Nursing Center team in 2013. She brings over 37 years of experience in health care food and nutrition services. Ms. Homes is a Dietetic Technician and is registered with the American Dietetic Association. She attended the State University of New York at Morrisville and earned an Associate in Applied Science in Food Service Administration with a Nutrition focus. Ms. Homes resides in Westernville, New York with her family.
Mr. Pohleven has over 20 years of experience in the construction management field. He has also worked as a heavy equipment operator. Mr. Pohleven joined St. Johnsville Rehabilitation & Nursing Center in 2014. He supervises the departments of laundry, housekeeping and grounds, and is responsible for the preventative maintenance projects of the facility. Mr. Pohleven resides in Herkimer County with his family.
Ms. Salomon is an RN with over 7 years of experience in the MDS field. Previously she held various positions in the Quality Management departments of 2 hospitals in addition to having a Quality Assurance background. She is also a Credentialed MDS Reviewer for long-term care reimbursement. She resides in Chadwicks with her husband and family.
Ms. Hojohn was raised in Canajoharie and began her career at St. Johnsville in 2011 after graduating from the Herkimer BOCES Practical Nursing Program. She also attended Hartwick College where she completed coursework in the Netherlands and South Africa. In 2014, Ms. Hojohn was promoted to Director of Delivery Reform Incentive Payment System Projects, Quality, and Interact Nurse Champion. She is currently working towards her RN at Excelsior College. Ms. Hojohn resides in Canajoharie with her family.
